Product description
Unity’s POS plans are similar to an HMO, but with added flexibility. Specifically, employees can receive care from in-network or out-of-network providers without a referral from the Primary Care Physician. However, care received out-of-network will be paid at a lower benefit level.
Provider network
Unity’s POS network includes the same providers as the HMO network. In addition, employees have access to health care providers throughout Wisconsin and the United States.
Guidelines
For groups with 2-5 enrolled employees, your company must be located in Unity’s 20 county service area to offer the POS product.
For groups with 6 or more enrolled employees, your company must be located in Unity’s 20 county service area to offer the POS product or any of the following multi-plan offerings:
