Unity PPO Plans

Product description
Unity’s PPO plans offer two levels of benefits: In-Network and Out-of-Network. With the PPO plan, your employees don’t need to choose a primary care physician. When services are received from In-Network providers, benefits are paid at a higher level.

Provider networks
To offer a statewide and national PPO network, Unity contracts with HealthEOS (Wisconsin) and MultiPlan (for other states). 

The HealthEOS network includes hospitals, clinics and physicians throughout Wisconsin. The MultiPlan network provides access to health care services nationwide.

Guidelines
To offer a Group PPO Plan:

• Your company must be located in Unity’s 20 county service area
• The product must be paired with a Unity HMO Plan, and at least 50% of employees must be enrolled in the HMO
• A total of 6 or more employees must be enrolled in any of Unity's plans
• Your employees must reside in Wisconsin (except for Dane County) OR in other states (except New York and Virginia)
