Unity offers a variety of HMO plans, including copayment, coinsurance and deductible plans. These plans allow your employees to select a Primary Care Physician and obtain all elective (non-emergency) health care services through a defined network of providers, hospitals and other health care professionals.
Provider network
Unity’s HMO provider network is built on the foundation of UW Health physicians, clinics and hospitals. As a company owned by the UW Health system, Unity offers strong discounts for medical services. In addition, Unity provides access to a broad network of local community providers.
Guidelines
For groups with 2-5 enrolled employees, your company must be located in Unity’s 20 county service area to offer the HMO product.
For groups with 6 or more enrolled employees, your company must be located in Unity’s 20 county service area to offer the HMO product or any of the following multi-plan offerings:
