Unity offers three different worksite wellness packages for employer groups with 50 or more employees to choose from based on budget and worksite wellness goals.

Classic

This package offers:

  • Online Health Risk Appraisal
  • Self-printed individual reports with results
  • Basic Health Risk Appraisal group summary report
  • Follow-up letter to participants with high risk factors

Gold

This package offers:

  • Everything in the Classic package plus:
    • Annual Health Risk Assessment (HRA) which includes the option of a paper or online questionnaire and biometrics onsite
    • Goal-oriented individual results review of HRA with motivational interviewing
    • One follow-up phone call to high risk HRA participants
    • Expanded Health Risk Assessment group summary report
    • e-Health tips for use in internal communications
    • One Lunch and Learn presentation

Platinum

This package offers:

  • Everything in the Gold package plus:
    • Health coaching for high risk HRA participants
    • Enhanced Health Risk Assessment group summary report
    • An additional Lunch and Learn presentation
    • Two incentive programs
