Fill It In and Win!

Get your Child Fully Immunized & Enter to Win a Prize

Will your child be fully immunized by the time he or she turns 2 years old this year? If so, Unity invites you to enter a drawing to receive great prizes!

Each year Unity sends children turning 19 months old an entry card for our Fill it in and Win! drawing. If your child recently turned 19 months old and has not yet received a Fill it in and Win! card, you may print an entry card  or contact Health Services at 888-829-5687 to request one. Once you enter the immunization information, make a copy of the completed card for your records and mail to:

Prizes
All cards fully completed and returned by June 15 will be entered in the Summer drawing for:

• Grand Prize--Summer Fun Kit (value $150)
• First Prize (1) --$100
• Second Prize (2)--$50
• Third Prize (3) -- $25

All cards fully completed and returned by December 15 will be in the Winter drawing for:

• Grand Prize--Winter Fun Kit (value $150)
• First Prize (1)--$100
• Second Prize (2) --$50
• Third Prize (3) --$25

Winners will be recognized in Unity's newsletter, UnityNews.

Official Rules: 

1. Cards are mailed when your child turns 19 months of age.
2. Your child must be a Unity member at time Unity receives completed card in order to be eligible.
3. Only your child addressed on the card is eligible to participate.
4. Child is eligible for only one drawing, either in June or December. Cards received after December 15, will be entered into the June drawing.
5. The June drawing will occur in the middle of June and will include completed cards received by June 15. The December drawing will occur in the middle of December and will include completed cards received by December 15. 
6. Completed cards have documentation (immunization date and clinic) that every immunization was given on or before the child’s 2nd birthday. For any immunizations not given, documentation of contraindication must be provided.
7. June grand prize is a Summer Fun Kit. December grand prize is a Winter Fun Kit. For each drawing, there is one first prize of $100, two second prizes of $50, and three third place prizes of $25. Cash prizes will be mailed to member.
8. Grand prizes are valued at $150 and determined on a yearly basis.
9. Prizes are non-transferable and cannot be exchanged for any other prize/merchandise.
10. Odds of winning are based on the number of entries received. Unity shall not be responsible for lost, late, misdirected, or illegible entries.
11. Winners will be recognized in Unity’s member newsletter.
