Active Living and Learning Program

Prevent diabetes or manage newly-diagnosed diabetes through education and lifestyle change

The Active Living and Learning* program is available to Unity members who have been diagnosed with one or more of these conditions:

Active Living and Learning members must fully participate in the program for 1 year and demonstrate a commitment to change their lifestyle habits (nutrition and exercise). The program is FREE to members who fully participate. There is an upfront cost of $250 as a "commitment" deposit, due upon program enrollment. The deposit is reimbursed at the end of one year if enrollees meet participation requirements.  To be eligible for reimbursement, Active Living and Learning enrollees must be Unity members from the start of the program through the payout date.**

Program description
Active Living and Learning is supervised by a Clinical Exercise Physiologist, a Registered Dietitian and a Behavioral Psychologist. The program runs for 1 year.  Classes meet in Madison.

Program features include:

Case managers monitor participants and provide follow-up through phone and email.

*The active Living and Learning Program is available through the UW Preventive Cardiology Clinic, 621 Science Drive, Madison, WI. This provider is an independent contractor and not employed by Unity Health Plans Insurance Corporation.

**Deposit reimbursement is dependent upon UW Preventative Cardiology Clinic’s assessment of participation criteria and Unity eligibility at the time of payout. Participation criteria include adherence to diet and exercise guidelines based on individual assessment; attending 2-hour group classes (1 hour of exercise, 1 hour of education); attending class once a week for the first three months; attending class once monthly for the program duration of 9 months; exercising a minimum of 4 times a week (in addition to the class); and keeping food and activity journals. Unity pays the program cost directly to UW Preventive Cardiology, based on member participation and Unity eligibility.  To be eligible to receive reimbursement and Unity program payments to Preventive Cardiology, the individual must be continuously enrolled as a Unity member from the start of the program through the time of payout.

The Active Living and Learning program is made available as a courtesy to our members and it is not part of your health insurance coverage. This program is not a contract between Unity Health Insurance and participants and may be discontinued or changed at any time without notice.
