How do I enroll in Fitness First?
A one-time sign-up in Unity's Fitness First Program is required before your workouts will count toward a reward. To enroll, complete the enrollment form online. Your Fitness First enrollment will be effective the first day of the month following receipt of your enrollment.
How do I know Unity received my enrollment form?
You will receive a welcome email indicating your Fitness First effective date, your reward selection, and the date your workouts will begin to count toward your reward.
Do I need to re-enroll each year in Fitness First?
No, you only need to enroll in the Fitness First program once.
How long does an exercise period need to be to qualify for Fitness First?
Each exercise period should be a minimum of 30 minutes.
Who is eligible for the $200 reward?
Both subscribers and their spouses or domestic partners, who are Unity members, and at least 18 years of age are eligible to work out for the cash reward. Together, you can earn a combined total of $200 each calendar year.
When I exercise at the health club, do I need to fill out both the log sheet at the health club and the tracking form?
If you choose to receive the cash reward, you must complete the log sheet or check in electronically at your health club. If you choose a prize reward, you should complete the tracking form.
What if I want to change health clubs or switch from a cash to a prize reward?
If you want to make any changes to your Fitness First enrollment information, please complete the Fitness First change form. If you’re changing health clubs, it is important to notify us so we can provide your name to your new health club.
Can my spouse or domestic partner and I earn prizes for the balance of the calendar year after we have earned the $200 cash reward?
Yes, but first you must change your Fitness First information from the cash reward to the prize reward. To be eligible to earn the cash reward the next calendar year, you must switch to the cash reward on the Fitness First Change Form a month prior to your next cash reward period.
When will I receive my reward check?
Cash reward checks will be issued six to eight weeks following the completion of your six consecutive months of qualifying workouts. If you complete your 60 times prior to the sixth consecutive month your check will still be mailed six to eight weeks after the sixth consecutive month.
Do I need to progress through each level of fitness prize?
No, simply complete the number of workouts necessary to earn the prize reward of your choice and indicate your selection on the tracking form when you send it to Unity. The tracking form must be redeemed within 12 months of the earliest workout date, but no later than 30 days after the last recorded date.
