Lifestyle Improvement through Nutrition & Exercise (LINE)

LINE is a reimbursement option for Unity members interested in enrolling in a lifestyle change or weight management program. Programs must include nutrition, physical activity and behavioral change components. Open to adults (over 18) and children ( 8-17 years old), the program must include these components and run a minimum of eight weeks for adults and six weeks for children.

LINE participants are reimbursed 50% of the program cost, up to a one-time reimbursement of $130.* To qualify for reimbursement, participants must:

Be a Unity member
Attend a pre-approved program or submit a proposal for another program that meets Unity’s requirements
Hold insurance coverage through Unity during the time of participation and at the time a reimbursement request is submitted
Participate in 75% of the program at a minimum
Complete a LINE program evaluation form
Complete a LINE program reimbursement form
Submit both forms to Unity within three months of the last program session

*Members may participate in LINE and Weight Watchers in the same calendar year, but each is a one-time, lifetime maximum benefit. Note that weight loss medication is not a covered benefit.
