Unity offers a variety of HMO plans to meet your needs. These plans allow your employees to select a Primary Care Physician and obtain most elective (non-emergency) health care services through a defined network of providers, hospitals and other health care professionals.

Eligibility Guidelines
- For groups with 2-5 enrolled employees, your company must be located in Unity’s 20 county service area.
- For groups with 6 or more enrolled employees, your company must be located in Unity’s 20 county service area to be eligible for the HMO plan and any of the following multi-plan offerings:

Provider Network
Unity’s HMO provider network is built on the foundation of UW Health physicians, clinics and hospitals. In addition, Unity provides access to a broad network of local community providers for companies and organizations outside of the Madison area. Learn more about the Unity provider network.
Health Savings Accounts
Health Savings Accounts (HSAs) are tax-exempt accounts established for paying qualified medical expenses. HSAs are designed to help individuals save for qualified medical and retiree health care expenses on a tax-free basis. An HSA must be used in conjunction with a qualified High Deductible Health Plan (HDHP), which includes Unity's Individual and Group HMO plan, and Group PPO and POS plans.
