Q. How do I select a Primary Care Physician (PCP)?
A. Look for a PCP who:

Use the Find a Doctor tool to look for a PCP in Unity’s Network. We recommend that you call the clinic to see if the physician you want is accepting new patients.

Q. How do I change my PCP?
A.  You can change your PCP by:

  1. Logging in to MyChart; or
  2. Contacting Unity Customer Service at 800-362-3310, Monday through Friday, between 7am and 5pm

Your PCP change will be effective the date you request it.

Q. What prescription drugs does Unity cover?
A.
If you have a prescription drug benefit through Unity, you can check Unity's Prescription Drug Formulary. The drug formulary  contains basic information on generic (Tier 1), Brand (Tier 2) and Non-formulary (Tier 3) medications.

Q. Do I need a referral for specialty care?
A.
 HMO members are not required to obtain a referral from the PCP prior to accessing specialty care if the specialist is participating, based on the PCP selection.  Please go to Find a Doctor to confirm that the specialist you want to see is participating for you. POS and PPO members should consult the Member Guide for specialty care information.

Q. What do I do if I need to go to an emergency room?

.  Call 911 or go to the nearest hospital.

Q. What do I do if I need urgent care?
A.
Call your PCP or go to a participating urgent care center.  Use Find a Doctor to help you locate a participating urgent care center.

Q. What do I do if I need to see a doctor and I’m traveling?
A.
Contact your PCP clinic. If it is an emergency, call 911 or go to the nearest hospital.

Q. How do I know how much I will pay for a health care service?
A.
The best way to determine this is by reviewing your Schedule of Benefits.  You may login to MyChart to view your Schedule of Benefits. The Schedule of Benefits lists any copayments, coinsurance or other amounts that you are be responsible for. 

Q. Who do I pay my copayment to?
A
. Some providers collect the copayment at the time of the appointment, while others will bill you later.  Do not send your payment to Unity.

Q. What services require prior authorization?
A
. You can review the services that require Prior Authorization for your plan in MyChart.
For behavioral health or chemical dependency services, call UW Behavioral Health (UWBH) at 800-683-2300 to discuss your needs. 

Q. How do I update my address?
A.
 Login to MyChart and complete the change of address form or call Unity Customer Service at 800-362-3310.
