It’s official! MyChart is now the secure online portal for all Unity members 18 years of age and older. We’re excited to have MyChart as part of our family and hope you’ve welcomed it into yours as well.

today! Complete the online form and an activation code will be mailed to your home within ten days. Once you receive your activation letter, follow the instructions to activate your account.
