HMO

HMO members must obtain elective (non-emergency) health care services through a defined network of physicians, hospitals and other medical professionals. Visit Find a Doctor to see Unity’s network of participating clinics and physicians. Each family member selects a Primary Care Physician (PCP). The PCP provides routine health care services and coordinates the use of specialty services.

PCP Rider for Out-of-Network Care
The Primary Care Physician (PCP) Rider is a benefit offered to employers located in Unity's Service Area (except Dane County) who select the HMO plan. The Rider allows members to choose a PCP who is not part of Unity's provider network. Learn more about the PCP Rider.

Prior Authorization
Some medical procedures require prior authorization. This means that your physician must obtain authorization from Unity before services are provided in order for you to obtain coverage. View the list of procedures requiring prior authorization.
