Medicare Select

Medicare Select provides supplemental health insurance coverage for individuals who are enrolled in Medicare Parts A and B. This plan covers specified hospital, skilled nursing facility, medical, surgical and outpatient services that are partially covered by Medicare.

Some medical procedures require prior authorization.  This means that your physician must obtain approval from Unity before you can receive coverage for these services.  View the list of procedures requiring prior authorization.

Apply for coverage
If you are interested in applying for Medicare Select, call your insurance agent or follow these steps:

  1. Complete the Enrollment Application .  You will need to select a Primary Care Physician (PCP) from Find a Doctor, select a payment option, and sign the application. Keep a copy of the application for your records.
  2. Read and sign the Medicare Notice .  Keep a copy of the notice for your records.
  3. Write a check for the first month’s premium. Unity will not cash the check unless your application is approved.
  4. Mail the Enrollment Application, Medicare Notice and your check to:
