PPO Plan

Unity’s Preferred Provider Organization (PPO) Plan provides coverage for employees who live in Wisconsin (except for Dane County) and other select states. It offers the flexibility of two benefit levels:  You may receive services from participating providers and receive a higher level of benefits, or you may receive services from non-participating and assume a greater share of the cost of your care. 

Unity has contracted with HealthEOS and PHCS MultiPlan to form a provider network for the PPO. HealthEOS providers include hospitals, clinics and physicians located throughout Wisconsin. Search for participating HealthEOS providers.

PHCS MultiPlan offers providers located throughout the United States. Search for participating PHCS MultiPlan providers.

Prior Authorization
Some medical procedures require prior authorization. This means that you must obtain approval from Unity before the procedure will be authorized for coverage. View the List of Procedures requiring prior authorization. Read the Notice to Enrollees and Claim Notice regarding the use of non-participating providers.
