Personal Options Individual Health Insurance

Personal Options is Unity's individual HMO policy that provides single and family health insurance benefits similar to those offered through an employer group plan.  With Personal Options, you select a Primary Care Physician (PCP) and obtain all elective (non-emergency) health care services through a defined network of hospitals, physicians and other medical professionals.

Some procedures require Prior Authorization. This means that your physican must obtain approval from Unity before the procedure will be authorized for coverage. View the list of procedures that require Prior Authorization.  

Unity's Service Area covers twenty Wisconsin counties. We provide access to a large network of providers within our Service Area. See the locations of our Primary Care Physicians.

A Plan to fit your budget
Unity offers a variety of Personal Options plan choices including:

• Copayment Plans-you pay a flat dollar amount for a medical service (e.g., a $25 copayment for a prescription drug)
  
  
• Coinsurance Plans-you pay a percentage of the cost of a medical service or supply (e.g., 20% of the cost of durable medical equipment)
  
  
• Deductible Plans-you pay a fixed dollar amount before Unity begins to pay benefits (e.g., you pay the first $500 of a hosptal inpatient stay; then Unity begins to pay for benefits)
  
  
• High Deductible Health Plans (HDHPs)-these plans are compatible with Health Savings Accounts (HSAs)

Benefit highlights

• Preventive care services are covered, including routine physicals, well-baby care, lab tests, x-rays and immunizations. Preventive care services may be subject to copayments, deductibles or coinsurance, depending on the plan you choose. Many immunizations are covered at 100%.
  
  
• You may purchase a maternity rider at the time of initial enrollment or within 31 days of marriage. (The maternity rider is not available with the HSA-qualified plans.)
  
  
• You have access to Fitness First, Fitness First for Kids and other popular Unity wellness programs. 
  
  
• The plan has a $2 million lifetime benefit maximum per member.
