Understanding Your Benefits

Unity's product offerings include:

Health Maintenance Organization (HMO) Plans
HMOs are based on the philosophy that a primary care physician (PCP) should coordinate all aspects of your health care needs, from routine check-ups to specialty care.  more

Point of Service (POS) Plans
POS plans are similar to HMOs, but with added flexibility. Specifically, you can obtain in-network care from participating providers or out-of-network care from non-participating providers. Care received from non-participating providers is covered at a lower benefit level.  more

Preferred Provider Organization (PPO) Plans
PPOs are designed for employers who have employees living in Wisconsin (except for Dane County) and other select states. Unity contracts with provider networks in order to offer in-plan PPO coverage locally, regionally and nationally. Although members have the flexibility of seeking services from providers who are not part of the PPO network, benefits for out-of-network care are paid at a lower level, and the member generally has higher out-of-pocket costs.  more

Health Savings Account (HSA) Eligible Plans
HSA plans are high-deductible plans designed to help you save for qualified medical and retiree health care expenses on a pre-tax basis.  more

Individual Health Insurance Plans (Personal Options)
This product is for individuals who do not have health insurance coverage through an employer group.  more

Medicare Select
This plan provides supplemental health insurance coverage for individuals who are enrolled in Medicare Parts A and B.  more
