Copayment and other possible charges
If you visit an emergency room, you will be responsible for the copayment shown in your Schedule of Benefits.
You may also have other charges, such as lab and X-ray charges. Ambulance transportation may require a copayment. Coverage is limited to usual, customary, and reasonable charges. See your Schedule of Benefits or your Certificate of Coverage (accessible through MyUnity).
Note: For My Choice members, the coverage for non-participating providers is limited to the Fee Schedule.
