Tools To Make The Most Of Your Health Plan

Accessing Care

Unity wants you to receive the care you need, when you need it. To support this, all Unity HMO and POS members select, or are assigned to, a participating Primary Care Physician (PCP). Your PCP is a physician who manages and coordinates your health care. Some procedures require prior authorization. This means that your physician must obtain approval from Unity before the procedure will be authorized for coverage.

Find a Doctor provides information about your physician and Unity's provider network. This tool helps you search for participating physicians and clinics.

Types of care and what to do
Primary Care (e.g., routine care such as physicals)
Make an appointment with your PCP or the Nurse Practitioner or Physician Assistant associated with your PCP.

Specialty Care (e.g., health care services provided by medical specialists)

HMO members:
Make an appointment with a specialist as directed by your PCP.  No referral is needed if you stay within your network. Whether a specialist is participating for you depends upon whether the specialist is in the same network as your PCP.

POS and PPO members:
For information about specialty care, see Information for POS Members  or Information for PPO Members  in the member guide. Referrals are not needed.

Urgent Care (e.g., minor cuts, sprains, ear infections) Medical problems like these are not life-threatening but do need prompt attention. Call your PCP or go to a participating urgent care center.

Emergency Care (e.g., heart attack, stroke, acute asthmatic attack, severe bleeding) Medical problems like these are life-threatening. Call 911 or go to the nearest hospital.

Away from Home Care (e.g., routine, urgent or emergency care) This is medical care needed while vacationing or while you are temporarily outside of Unity’s service area. Contact your PCP clinic. If it is an emergency, call 911 or go to the nearest hospital.
