If you are responsible for out-of-pocket expenses such as a copayment, coinsurance, deductibles or amounts in excess of a usual, customary and reasonable charge, Unity will send you an Explanation of Benefits (EOB). See a sample Unity EOB. ![]()
The EOB outlines the total cost of the medical services received, the amount covered by Unity and the amount that you are responsible for paying.
For confidentiality purposes, Unity mails the EOB to the person who received the medical service. That means that a child will also receive EOBs. Sign up to receive your EOB electronically.
It is important to note that an EOB is not a bill. The provider who performed the service will send you a bill if you are responsible for an unpaid balance.
If you have questions about an EOB, please contact Unity Customer Service.
