What you can expect when you join Unity

Select a Primary Care Physician (PCP) to manage your care. If you do not select a PCP, one will be automatically assigned to you. Your PCP may be a:

• Family Practice Physician
• Family Practice with Obstetrics
• General Practice Physician
• Geriatric Physician
• Internal Medicine Physician
• Pediatrician
• Obstetrics-Gynecology Physician

Make an appointment
Contact your PCP when you need non-emergency care. Keep in mind that you may be able to schedule an earlier appointment if you see a Nurse Practitioner (NP) or Physician Assistant (PA).

Your PCP should:

• Know your medical history and coordinate your health care needs
• Monitor your care if you have a chronic medical condition (e.g., diabetes, asthma)
• Recommend that you seek age-appropriate preventive health services (e.g., routine physicals and health screenings)

Prepare for your appointment
Use Healthwise® to print out and complete a handy checklist to help you prepare for your appointment.

Changing your PCP
If you want to change the PCP listed on your ID card:

• Login to MyUnity and complete the online PCP change form; or
• Call Unity Customer Service at 800-362-3310 Monday through Friday between 7am and 5pm

Know your Rights & Responsibilities
As a Unity member, you have the right to choose a personal physician from Unity’s PCP network. You also have the responsibility to establish a relationship with this physician.  See Member Rights & Responsibilities.
