Collection of Social Security Numbers
In accordance with Federal law, the Centers for Medicare and Medicaid Services (CMS) is required to obtain social security numbers for individuals who are part of a group health plan in order to properly coordinate payments with other insurance benefits.
Due to this requirement, Unity is asking subscribers, their covered spouses, and other Medicare beneficiaries to provide us with their social security number. Unity must provide this information to CMS. We do not use social security numbers for any other purpose.

Security
Our Web Site requires users to provide unique identifiers, such as subscriber identification numbers, to log into some of the web site pages.
