Provider Pay for Performance

Overview
Unity’s Pay for Performance Program offers Primary Care Physicians in Unity’s regional service area financial incentives for providing certain lab tests and medical screenings for identified members.

Program goal
The main goal of this program is to improve the health of our members by making sure they receive the recommended screenings and lab tests.

Key areas of focus
The main areas of focus are diabetes, chlamydia, colorectal, and body mass index (BMI) screenings.   Each year, areas of focus are evaluated and may change based on our current quality initiatives.

How the program was developed
Unity worked in collaboration with physicians in our regional service area and UW Health. Together, we reviewed clinical practice guidelines, established appropriate measures and assessment tools and set priorities.

How Pay for Performance works
If you are identified as having eligible patients who meet program requirements, you will be contacted periodically with instructions. Eligible patients needing certain lab tests or medical screenings will also be contacted with instructions.

Financial incentives
Please submit the appropriate information according to the area of focus by the date indicated. Incentives vary by area of focus.  The method of submission is through MyUnity for Providers.
