All Unity Members* have medical benefits for Temporomandibular Disorders (TMD/TMJ). Search By Specialist for practitioners that provide these medical benefits related to teeth and the mouth.

Certain Unity Members have dental benefits (i.e., teeth cleanings, restorative and/or orthodontia services). To find who administers your dental benefit select your provider network in the chart below.

*BadgerCare Plus members may receive dental services from any dentist who will accept the Forward ID card/ForwardHealth ID card. These dental services are provided by the State, not Unity.

UW Health Network Members

Community and Unity Network Members

Momentum Insurance Plans, Inc. administers dental benefits for members who have selected a PCP in the UW Health Network. To find out if your dentist is in the Momentum network please access their website at momentumplans.com/unity or call toll-free 855-729-6569 or locally 608-729-6569.

Delta Dental administers dental benefits for Community and Unity Network members. Through Delta Dental you have access to either the Delta Dental PPO network or the Delta Dental Premier network. To find out if your dentist is a Delta Dental provider please access Delta's website at deltadentalwi.com and select either Delta Dental Premier or Delta Dental PPO as your dental plan, or call 800-236-3712.

All follow up care related to accidental injury to teeth must be received by a provider in the dental network of your dental administrator.

Behavioral Health Services:
To receive behavioral health services you must call UW Behavioral Health at 608-233-3575 or toll free at 800-683-2300 to obtain prior authorization of all assessments and treatment.

Chemical Dependency Services:
Members who have a PCP in Dane County
(Excluding those PCP’s in Black Earth and Cambridge and BadgerCare members)
To receive chemical dependency services you must call Gateway Recovery at 608-278-8200 or toll free at 800-785-1780 to obtain prior authorization of all assessments and treatment.

Members who have a PCP outside of Dane County
All chemical dependency services must be prior authorized by calling UW Behavioral Health at 800-683-2300 or 608-233-3575 for prior authorization of all assessments and treatment.

BadgerCare HMO members
Behavioral Health and Chemical Dependency Services:
Standard and Benchmark Plan members who have a PCP in Dane County

To receive behavioral health or chemical dependency services you must call Journey Mental Health at 608-280-2720 to obtain prior authorization of all assessments and treatment.

Standard and Benchmark Plan members who have a PCP outside of Dane County and All Core Plan members
To receive behavioral health or chemical dependency services you must call UW Behavioral Health at 608-233-3575 or toll free at
800-683-2300 to obtain prior authorization of all assessments and treatment.
