ELIGIBLITY: SWEEPSTAKES IS OPEN TO UNITY MEMBERS 21 YEARS OF AGE OR OLDER. Employees of Unity Health Insurance and its respective parents, divisions, affiliates, subsidiaries, members insured through the State of Wisconsin health insurance program, BadgerCare Plus members, and their immediate family members (parent, child, sibling and spouse), persons living in the same households of each such employee or member (whether related or not), are not eligible. Void where prohibited by law.
There are two ways to enter for a chance to win:
1. Automatically: Unity members 21 years of age or older that register for MyUnity between 12:01 AM on 10/15/09 and 11:59 PM on 12/31/09. Members registered for MyUnity before 10/15/09 are not eligible for this drawing.
2. By Mail (without registering for MyUnity): To enter via postal mail, send a 3” x 5” card with your name, address and daytime and evening telephone numbers postmarked by 12/24/09, to be received no later than 12/30/09, to: Unity Health Insurance MyUnity Sweepstakes, 840 Carolina Street, Sauk City, WI 53583. Only one handwritten entry per envelope.
Prizes: Two (2) tickets to one (1) of the following University of Wisconsin sporting events: Men’s Basketball, Women’s Basketball, or Men’s Hockey and a $50 gift card to Bucky’s Locker Room. Available games are determined by Unity Health Insurance. The odds of winning depend upon the number of automatic and mailed entries received. Winners will be notified by mail and instructed to complete the required Affidavit of Eligibility, Liability and Publicity Release (where legal). If the Affidavit, properly executed by a potential winner, is not returned within 10 business days of the date on the notification, or if the potential winner is found to be ineligible or does not comply with these Official Rules, the Prize will be forfeited and awarded to an alternate entrant. A potential winner who is disqualified for any reason will not be entitled to any form of compensation. No substitution of prize permitted. Federal, state, and local taxes, and any expenses relating to the acceptance and use of the prize not specified are the responsibility of the winner.
Sweepstakes Drawing: A random drawing will be conducted on or about 1/11/10 from among all eligible entries received. The randomly selected winner will be notified by mail/email to the address of the primary Account holder or the address indicated on the mail-in entry within 10 business days of the winner selection process. Any prize not claimed or returned as undeliverable may be awarded to an alternate entrant.
Privacy: Your participation in this Sweepstakes constitutes your consent for Sponsor and the Administrator to use your personal information only for the purpose of administering the Sweepstakes and awarding the prizes, unless you have opted to receive e-mail communications from Sponsor. General: All entries are subject to verification prior to awarding the prize. No automated entries made using any script, macro, sweepstakes service, automated entry software, or facsimiles will be accepted. By entering, participants release and hold harmless Sponsor and Administrator and their parents, subsidiaries, affiliates, directors, officers, employees and agents from all and any injury, damage, loss, expense, accident, delay, inconvenience, or irregularity arising out of or relating to any prize won in this Sweepstakes, even if caused by the negligence of the Sponsor. Sponsor is not responsible for printing or typographical errors in any Sweepstakes-related materials; for lost, stolen, late, misdirected, damaged, incomplete, illegible, or postage-due mail, or for transactions that are lost or fail to enter the processing system, are processed, reported, or transmitted late, or are lost for any reason including computer, telephone, paper, or any other malfunction including human error. Sponsor, at its sole discretion, reserves the right to cancel or modify this Sweepstakes, in whole or in part, at any time without notice if fraud or technical failures (including but not limited to infection by computer virus, bugs, tampering, unauthorized intervention, or any other causes beyond the control of the Sponsor and the Administrator) or any other reasons whatsoever interfere with its conduct as contemplated by these Official Rules.
If the Sweepstakes is cancelled for any reason before completion of the Promotion Period, all automatic and mailed entries received up to the point of termination will be included in a random drawing to award the Prize. Submitted materials become the sole property of Sponsor and will not be returned. By entering, you accept and agree to these rules and the decisions of Sponsor and the Administrator which shall be final and binding in all respects relating to this Sweepstakes.
ANY ATTEMPT TO DELIBERATELY DAMAGE THE WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, UNITY HEALTH INSURANCE RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES (INCLUDING ATTORNEY’S FEES) TO THE FULLEST EXTENT OF THE LAW.
Force Majeure: The failure of Sponsor to comply with any provision of these Official Rules due to an act of God, hurricane, war, fire, riot, earthquake, terrorism, act of public enemies, actions of governmental authorities – outside the control of either (excepting compliance with applicable codes and regulations) – or other force majeure event will not be considered a breach of these Official Rules.
